Job summary
We are currently recruiting to our Safeguarding Families team and are looking for an enthusiastic, highly motivated and innovative individual to join our dynamic and forward thinking team as Safeguarding Adults Practitioner.
The post holder will be a highly visible experienced practitioner, providing, with support from the Safeguarding Adults Lead specialist knowledge and advice to all staff in the Trust and the community in all areas of Safeguarding Adults. With support from the Safeguarding Adults Lead you will provide the professional interface between hospital and community services with respect to the welfare of adults, providing a robust and safe communication system for the effective and timely flow of information between hospital and community health professionals and other agencies as appropriate.
The Safeguarding Adult Specialist Practitioner will work closely with the wider safeguarding and complex health teams to ensure a holistic approach to the care of the most vulnerable patients and their families in delivering safeguarding advice, support, training and supervision. The hospital provides an efficient and effective liaison and expert screening system and you will be sharing information to the appropriate community staff where there are specific health, social or safety concerns.

Main duties of the job
Apply knowledge to support positive clinical outcomes and compassionate care for patients who are experiencing abuse and/or harm from others.
Promote best practice and evidence based approaches to safeguarding adults at risk
To support the co-ordination of care and safety planning for people who require support to ensure there are safe and to support clinical teams to follow these and respond to escalation in concerns
To support the delivery of training and education to staff when they work with adults at risk.
Experience
Essential
Proven post registration experience of working within the health or social care sector
Experience in working with others as part of a team.
Experience of working within the safeguarding arena
Desirable
Experience of working within acute and community settings.
Experience in of audit and monitoring processes
Qualifications
Essential
Relevant health or social care qualification
Relevant Safeguarding post basic training or equivalent experience
Desirable
Relevant teaching qualification or working towards
Educated to 1st level degree.
Knowledge
Essential
Experience in inter-agency work
Evidence of on-going professional learning
Knowledge of adult's services
Knowledge of legal framework for information sharing
Understanding of the NMC Codes of Conduct
Desirable
Knowledge of legislation relevant to safeguarding adults
Knowledge of evidence based safeguarding adults practice
Experience in education and training
Knowledge of adult's rights
